Case management type position is integral in providing primary care services within an interdisciplinary team setting. Enroll members (performing initial Hx and PE) and visit them in their homes periodically, as well as work in conjunction with primary MD and team members to manage chronic diseases and provide education. Prefer 2 more more years experience but will consider new grads.
